If you're looking down at your arthritic knee and seeing only a worn-out joint with eroding cartilage, you're thinking about your knee the wrong way. There are two problems with this line of thinking. First of all, the structural changes that commonly take place in arthritis, things such as cartilage loss, meniscus tears, and bone spurs, can all be found in people with no knee pain. Consider the study that looked at 319 subjects between the ages of 25 and 74 with knee arthritis on x-ray - and found that only 47% had knee pain. Put another way, 53% of these people were walking around with knee arthritis and no pain. Yet another study looked at 49 subjects over the age of 45 with no knee pain or arthritis, and found that 76% of them had meniscus tears! The second problem with focusing on the structural changes in knee arthritis is trying to figure out what's causing your pain. Most of the numerous structures in your knee have nerve pain fibers going to them, so good luck trying to pinpoint which one(s) are the exact source of your pain. In fact, about the only knee structure we can safely rule out is the articular cartilage that can be seen wearing out on x-rays! Knee cartilage itself actually has no pain fibers going to it, and therefore cannot produce any pain. So what now? Treat Your Own Knee Arthritis takes a new approach to an old problem. Instead of worrying about structural problems (that studies show people can live just fine with) you will learn how to fix the functional problems found in knees with arthritis. And as the research shows again and again, if you improve functions such as the strength and proprioception of your knee, the pain goes away. Based entirely on randomized controlled trials, Treat Your Own Knee Arthritis is a simple, yet effective program that can be done in the privacy of your home with minimal cost or equipment. Exercise sheets are also provided to help guide you step-by-step through a six-week program.

Early in its development, the subject matter of any field of surgery is too ill-defined and opinions are too fluid for the production of a book on the subject to be possible. Late in its development, controversy is at an end, and although it is still possible to produce a textbook, it is too late to produce a book that might stimulate discussion and crystallise ideas. This book has that objective, it being the Editor's view that the field of the surgical treatment of arthritis of the knee had reached an appropriate intermediate stage in 1978 when this text was written. Three broad issues stand out as being in need of resolution before the optimum form of surgical treatment for a given knee can be defined more convincingly than is possible at present: Firstly: What symptomatic and physical features of the knee are to be recorded pre- and post-operatively, upon the basis of which comparisons can be made between the results obtained by two different surgeons or with two different tech niques. The resolution of this issue requires general agreement not only upon what features of the knee should be recorded but, crucially, upon how these features should subsequently be presented so as to characterise a particular group of knees.

In my book: "The Fit Arthritic: Fighting Knee and Hip Arthritis with Exercise" I will describe how someone with arthritis can start to take control of their pain and improve their health. I encourage collaboration with a health professional and will help my readers to learn more about osteoarthritis and self-treatment options. I detail many medications in the book and hope that my readers will be able to reduce their use of prescription and over-the-counter pain medications. I define fitness for someone with arthritis and help the reader to plan and exercise their way to less pain and improved health. My goal is that those affected with osteoarthritis can be come a "fit arthritic" with me. From the Foreword: "I am now in my mid-forties and pleasantly enjoying mostly pain-free days. I used to consume more than 2000 ibuprofen tablets yearly and now I rarely take them for knee pain. I can walk for extended distances and finished a day-long hike in Yosemite National Park this year. I am finally at a healthy weight around 175 pounds. I have discovered many principles that have helped me to become more active, suffer from less pain, and yes, consider myself fit once again." ALAN KELTON M.D. is a practicing internal medicine doctor. He is an Assistant Clinical Professor of Medicine at UCSF Fresno. He is listed as one of "Americas Top Physicians"for 2007 and was selected by physicians in the Fresno, California area as a "Best Doctor" and he has been recognized as a "Top Ten"physician for quality of care for two years in a row. "Take away my knee pain and give me better motion." This is what the arthritic patient expects from a Total Knee Arthroplasty (TKA). By virtue of standardization of the TKA procedure, surgeons can nowadays solve the pain issue for the majority of the patients. Restoration of function is a goal of a different order and forms the scope of this book. The editors confronted today's leading knee surgeons with the limitations of current surgical techniques and technology. They challenged them to define new thresholds of functional capacity after Total Knee Arthroplasty. "A Guide to Get Better Performance in TKA" describes the cutting edge in surgical techniques, prosthetic design and achievement of excellent function for these patients.
